9. Troubleshooting
Target audience: All users
Description: A self-help index that maps common problems to their causes and fixes. Each entry links to the page or task where the feature is documented in full.
Find your symptom in the index, then follow the link. If nothing here helps, see 7.3 Help and Support.
9.1. Login & access
I cannot sign in
Possible cause 1: Display name used instead of username. CQA signs you in with your username (for example, A1234), not the display name shown in the app (for example, "Agent 1234"). A display name is rejected as invalid credentials.
Solution: Enter your username. If you do not know it, ask your CQA administrator; it is listed in the Username column of User Management.
Possible cause 2: Wrong or missing Tenant ID. The login page asks for your organization's Tenant ID as well as your username and password.
Solution: Enter the Tenant Name or ID provided by your administrator, then your credentials. See 2.2.1 Sign in.
Possible cause 3: Account locked after too many failed attempts. After 5 failed sign-in attempts within 30 minutes, the account is locked, and it does not unlock automatically after a set time.
Solution: Reset your password with Forgot Password? — a successful reset also unlocks the account. See 2.2.3 Forgot Password. If your organization uses SSO / IAMIX, reset through your identity provider instead.
The invitation link is invalid or expired
Possible cause: Invitation links are valid for 72 hours, for secure onboarding.
Solution: Ask your CQA administrator to invite the same email address again in User Management. The User Management page does not have a resend action, but inviting the same email again creates a fresh 72-hour invitation link (and applies the role and username from the new invite). You do not need to contact CQA Support. See 2.2.2 First-time login and Invite a user.
My password is rejected or the reset link does not work
- Reset link expired or already used: Reset links expire after 30 minutes. Request a new one from Forgot Password?.
- No reset email: Wait a few minutes and try again. Repeated requests in a short window may be rate-limited.
- Password rejected: Follow the Password Policy and the rule checklist on the Accept Invite and Reset Password screens; the checklist shows which rule is not met as you type.
A page is missing from my menu
Possible cause 1: Your role does not include it. For example, Usage & Limit and Policy (in the footer of the left navigation) are Admin-only; Agents do not see Test Evaluation; most Manage pages are for Admins.
Solution: Check What each role typically sees. Ask your administrator if you need access.
Possible cause 2: The page is not part of your account setup.
Solution: Contact your Customer Success Manager.
9.2. Data visibility
A dashboard or list shows no data
Applies to WORK › Dashboard, Analysis List, and AI Insight.
Possible cause 1: Filters are too narrow. The date range, agent, or Quality Profile filter may exclude all data.
Solution: Reset the filters or widen the date range. On a dashboard, deselect the Quality Profile to see all profiles (3.8 Dashboards).
Possible cause 2: Your access is restricted by metadata. Your account may be limited to certain metadata values (for example, one Region or Campaign).
Solution: You only see data within your access rights. Ask your administrator to check your Access Metadata in User Management.
Possible cause 3: Nothing has been analyzed yet. If no interactions were analyzed in the period, there is nothing to show.
Solution: See Incoming calls are not being analyzed.
A metadata field is missing from filter dropdowns
Possible cause: The field exists but is not enabled for filtering.
Solution: An Admin goes to Manage › Data Integration › Metadata and turns on Enabled and Filter Option for the field. See Configure how a field behaves.
9.3. Analysis & automation
Incoming calls are not being analyzed
Possible cause 1: No Assignment Rule matches. The call's metadata does not match any active Rule-Based Assignment.
Solution:
- Go to Manage › Configuration › Assignment Rule.
- Check the rule is active and review its conditions (for example, "Queue Name IS Support").
- Check the incoming call actually carries that metadata value. If it does not match exactly, the call is not analyzed.
- Check the rule's analysis percentage — only that share of matching calls is sampled.
Possible cause 2: Smart Assignment found no topic match and has no fallback. If no AI Match Profile meets the confidence threshold and the rule has no Default Profiles or Fallback Profile, nothing is scored.
Solution: Add a Fallback Profile, broaden the Confidence Threshold, or improve the Topics on your profiles. See Smart Assignment profile roles.
Possible cause 3: The conversation cap is reached. When the account hits its daily, weekly, or monthly cap, further interactions are not analyzed until the period resets. API ingests return CONVERSATION_CAP_EXCEEDED.
Solution: An Admin checks Usage & Limit, in the footer of the left navigation (7.1) and contacts the CSM to change the cap. To get warned in advance, turn on Usage Approaching Plan Limit and Max Analysis Cap Reached in Alert & Notification.
A call was scored against an unexpected Quality Profile
Possible cause: Topic matching, Confidence Threshold, Default Profiles, or Fallback Profile settings on the Smart Assignment rule — or several rules matching the same call.
Solution: Review the Topics on the relevant Quality Profiles. In the Assignment Rule, check AI Match Profiles, Confidence Threshold, Default Profiles, and Fallback Profile. Tighten or broaden the threshold, or adjust topics, so the intended profiles match. Remember that every matching rule contributes its profiles (How rules are evaluated).
I cannot add more topics to a Quality Profile
Possible cause: The profile already has the maximum of 10 topics.
Solution: Remove a topic before adding a new one. See Topics.
9.4. Test Evaluation
A Test Evaluation shows Partial Success or Failed
Test Evaluation is available to Supervisors and Admins.
Possible cause: Some rows in the uploaded file have invalid or unreachable audio URLs, or are missing mandatory data. On free plans, the test-evaluation limits on audio minutes and text words also apply.
Solution:
- Open the test evaluation from the list in WORK › Test Evaluation.
- Check the count of successful and failed items.
- Filter the list to find the failed recordings and check their URLs in your source file. URLs must be publicly accessible.
- On free plans, an Admin can check the limits under Free Plan Limits in Usage & Limit.
9.5. Disputes & scoring
I cannot change a score while resolving a dispute
Possible cause 1: The scorecard is in view mode.
Solution: Click Edit on the Interaction view to enable score changes and dispute resolution. See Resolve a dispute.
Possible cause 2: Your role cannot edit scores. Agents can raise disputes but cannot change scores; Supervisors can.
A critical KPI failed and zeroed the score
Possible cause: This is how criticality works. The effect depends on the criticality level set on the KPI.
Solution: Check the KPI's Criticality in the Quality Profile (KPI settings):
- Category — only that category's score becomes 0.
- Sub-category — only that sub-category's score becomes 0.
- Profile — the whole interaction score becomes 0.
9.6. Configuration
"KPI title already used" error
Message: "Another KPI in this quality profile already uses this title. Titles must be unique within a profile."
Possible cause: Another KPI in the same profile has the same title. The check ignores case and leading or trailing spaces, so "Greeting" and " greeting " count as the same.
Solution: Rename one of the KPIs. The same rule applies to the KPI column in Excel uploads. See KPI settings.
A Knowledge Base document failed or cannot be deleted
- Status is Failed: Check the file is PDF, DOCX, TXT, or MD and no larger than 10 MB, then upload again. An account can hold up to 10 documents.
- Cannot delete: A document linked to KPIs cannot be deleted. Click its Linked KPI's count to see which KPIs use it, unlink it from those Quality Profiles, then delete.
- Cannot link to a KPI: Only Indexed documents can be linked. Click Refresh data to update the status.
See 4.2 Knowledge Base.
Analysis failed with a Tool Integration error
When a KPI uses Tool Integration and the tool cannot be resolved, the analysis fails rather than scoring without verification.
Reason | Typical cause | What to check |
|---|---|---|
TOOL_METADATA_MISSING | A metadata key mapped in the tool URL is missing or empty on the interaction. | Ingestion mapping, the interaction's metadata, and the tool's URL parameter mapping. |
TOOL_UNAVAILABLE | The tool returned an HTTP error, is disabled or missing, or the selected response fields are missing from the response. | Tool URL and credentials, field paths, and network allowlists. Use Test connection on the integration. |
See Failure behavior and Register a REST API.
Alert emails were not delivered
Possible cause 1: Enable Notifications is off, or the event is turned off.
Solution: Check both in Manage › Configuration › Alert & Notification and click Save Changes. See Turn on notifications.
Possible cause 2: Some recipients have no email address, or an address is wrong. Alert History shows Failed or Partially Delivered.
Solution: Open Alert History, click the row's Recipients, and correct the addresses or user emails. See Review alert history.
9.7. Language, transcripts & AI Suggestions
Language and transcripts
I do not see a language toggle on the Interaction view
- Not available: Native Language Support is not available on your account. Ask your administrator to coordinate with the Exotel delivery team.
- Older interaction: The interaction was processed before Native Language Support was enabled. Only interactions analyzed after enablement have dual-language transcripts; earlier ones show the English transcript only.
The detected language does not match the spoken language
The language detection model identifies the dominant language in the audio. With heavy code-switching (for example, Hindi and English mixed), it may pick the most prevalent language. This is expected for mixed-language conversations. If detection is consistently wrong for single-language calls, report it to CQA Support with the Interaction ID.
AI Suggestions
I do not see an AI Suggestion for a failed KPI
Possible cause | Solution |
|---|---|
AI Suggestions are not available on your account. | Ask your administrator to coordinate with the Exotel delivery team. |
The interaction was processed before AI Suggestions were enabled. | Only interactions analyzed after enablement have AI Suggestions. |
The KPI is a Text type (data extraction). | Suggestions are generated only for scoring KPIs (Yes/No, Selection, Rating). |
The KPI received full marks. | Suggestions appear only when the score is below the maximum. |
An AI Suggestion seems inaccurate or irrelevant
Possible cause | Solution |
|---|---|
No KB document is linked to the KPI. | Without one, CQA gives generic best-practice tips. Link Knowledge Base documents to the KPI. See 4.2 Knowledge Base. |
The document is outdated or incomplete. | Update it with current, detailed procedures. The more specific the document, the better the suggestions. |
Genuine inaccuracy. | Use the thumbs-down button. This feedback helps improve AI Suggestions. |
See AI Suggestions.
9.8. Reports, exports & jobs
A Report History run failed
Go to WORK › Report History, set the Status filter to Failed, and read the status shown in the table to tell failure types apart.
Status | Meaning | What to do |
|---|---|---|
Handler Failed | The data processing step failed — for example, the source was unreachable or a file had the wrong format. | Check the source connector and file format in Manage › Data Integration. |
Delivery Failed | Processing succeeded, but every delivery destination failed. | Check the destination connector (Email, S3, or SFTP) and run Test Connection. |
Partial Success | Processing succeeded, but some destinations failed. | Fix the failed destination. |
Exports and scheduled reports
Problem | Possible cause | Solution |
|---|---|---|
An export job was created but no report was generated. | The start date is in the future. Scheduled jobs generate reports only after the start date. | Check the job's start date in Set Schedule. Check the run in Report History. |
The report is empty. | No analyses matched the selected Quality Profile and filters in the date range. | Confirm the profile has completed analyses in the period, and review the filters in Select Data. |
A scheduled job stopped running. | The job's end date has passed, so its status is Expired. | Create a new export job with a new end date or Ends: Never. See Job statuses. |
I cannot pick a longer date range for a one-time export. | One-time exports allow at most 31 days. | Split the export into several ranges, or set up a recurring export. See Export job data range. |
A large report arrived as several files or links. | Each exported file holds up to 2,000 rows; larger reports are split into parts and bundled into one ZIP. | Open the ZIP from the delivery email and extract the part files. |
An email download link no longer works. | The link has expired: links from a scheduled job's email are valid for 72 hours; links from a one-time Email Me export are valid for 7 days. | Export again, or run the job again. |
I cannot find my exported report. | — | Every export, one-time or scheduled, is listed in WORK › Report History. Search by job name. |
9.9. Data Integration and API
Connector Test Connection or Test Run fails
- Test Connection fails: CQA cannot reach or authenticate to the source or destination. Check the host, bucket or path, credentials, and network allowlists.
- Test Run fails: Look at the stage that failed — for example, Authentication, File Discovery, File Validation, Data Discovery, Call Stats Fetch, or Recording Processing. Test Run also fails if there is no file to analyze.
- No New Recordings: Not an error. Connectivity works, but every call found was already processed.
The API returns an error
Check the error.code and message in the response, and keep the request_id for support.
Code | Typical fix |
|---|---|
UNAUTHORIZED | Check the X-API-Key header. A revoked key can stop working at any time within 24 hours of revocation. See 5.4 Access API. |
VALIDATION_ERROR | Fix the field named in message — for example, a missing required field, more than 50 metadata keys, or a date range over 31 days. |
DUPLICATE | An interaction with this external_interaction_id already exists. Use a new ID. |
RATE_LIMIT_EXCEEDED / TOO_MANY_JOBS | Slow down, or wait for running file jobs to finish. See 6.8 Limits. |
CONVERSATION_CAP_EXCEEDED | The account's cap is reached. See Incoming calls are not being analyzed. |
Full list: Error codes.
Callbacks are not arriving
Possible cause: Callback delivery is not turned on for your account, so callback_url values are stored but not called.
Solution: Contact your Customer Success Manager. See 6.7 Webhooks / callbacks.
9.10. PII Redaction
I see raw PII in transcripts even though redaction should be active
Possible cause | Solution |
|---|---|
PII redaction is not active for your account. | Contact your Customer Success Manager to verify it is active. |
The interaction was processed before redaction was enabled. | Redaction applies only to newly processed interactions. Earlier interactions keep their original content. |
The PII category (for example, Person Name) is not active for your account. | Ask your CSM which categories are active, and request changes if needed. |
Detection missed the data. | Detection is automated and can miss unusual formats, code-switched languages, or ambiguous context. Report the Interaction ID to CQA Support. |
PII is redacted in the transcript but appears in a downloaded report
This is not expected; all surfaces should show the same redaction. Report it to CQA Support with the Interaction ID and the field where the difference appears.
See 4.7 PII Redaction.
